by conducting an
ElderCare Assessment™
of the client's physical, emotional, psychological, social, and
spiritual needs. Together with the caregiver and/or family
members, we then custom tailor and implement the
ElderCare Plan™
with the elder care services that best accommodate the
patients and their family’s unique needs Your geriatric care manager
will work closely with you and other members of the
multidisciplinary care team to
continually monitor and update the
ElderCare Plan™
as your loved ones needs change.

The
ElderCare Assessment
™
can performed at your
home, in a hospital or nursing home or in any setting you are
most comfortable. Scheduling is easy and at your convenience.

Call us to coordinate a date and time that is best for you.
Telephone assessments can be performed when long distance
family members are not in the area or other circumstances
prevent a face-to-face meeting.

The
ElderCare Assessment™
will take anywhere from one hour to three hours to complete.
We will be gathering a lot of information including but not
limited to, health history, social history and other
demographic information. In order to provide you with
a comprehensive, objective and professional evaluation, we
will then meet with the patient.

After the visit is complete, the Geriatric Care Manager will be in
contact with you and you will receive a written
ElderCare Plan™
that contains his or her
recommendations and objectives.

Together with you, the Geriatric Care Manager can implement some or all
aspects of the ElderCare Plan™.

The Care Manager will continue to work with the patient,
caregiver, family members and other professionals to provide
advocacy, emotional support, counseling, guidance and
further suggestions or recommendations. You will
decide the level of involvement you would like your
Geriatric Care
Manager to have.
